By completing this survey, you will be entered into our prize drawing and eligible to win a $100 Canadian Tire gift card.

The prize winner will be randomly selected and contacted via email by June 30th, 2025. Winners who do not respond to our email within 5 working days may forfeit their gift card. 

To be eligible to win, you must complete the survey by May 30th, 2025, which may include submission of your personal information, governed by our privacy policy. 

By participating in the survey raffle, you release the Champions for Life Foundation from any liability.
